Consent to the processing of personal data

By ticking the box “I give my consent to the processing of personal data” in the request form on flotservice-murmansk.ru, I freely, of my own will and in my own interest give my consent to FlotService LLC (ООО «ФЛОТСЕРВИС»; INN 5190240166, OGRN 1265100000439; Office 304, 17 Sofyi Perovskoy St., Murmansk, Murmansk Region 183038, Russia; the “Operator”) to process my personal data on the following terms.

1. Data covered

Name, phone number, company, vessel name, vessel length, port, preferred inspection format, comment text, page language, and a technical identifier based on the IP address, which is used only to protect the form from spam and is kept for up to 10 minutes. If I send the request as a message from my own e-mail app (fallback option), the Operator also receives my e-mail address.

2. Purposes

Reviewing the request and quoting the vessel inspection; contacting me by phone and e-mail about the request; preparing and concluding a service contract.

3. Operations

Collection, recording, systematisation, accumulation, storage, clarification (updating, changing), use, deletion, destruction — with and without automation tools. Disclosure of the data to an unlimited number of persons is not permitted. The request is delivered to the Operator's corporate e-mail (flotservicecompany@mail.ru, Mail.ru service); the Website is hosted by the Russian hosting provider Timeweb.

4. Term and withdrawal

This consent is valid until the purposes of processing are achieved, but no longer than 3 years from the date it is given if no contract is concluded. I may withdraw it at any time by writing to flotservicecompany@mail.ru or to the Operator's address. After withdrawal, the Operator stops processing and destroys the data within 30 days, unless otherwise required by law.

5. Confirmation

I confirm that I have read the Operator's Personal Data Processing Policy and that the data I provide is accurate. This consent is given under Article 9 of Russian Federal Law No. 152-FZ “On Personal Data” of 27.07.2006.
